Terms & Conditions
1. Agreement
These Terms and our Privacy Policy form a binding agreement between you and ForLife Global Pty Ltd (“ForLife”, “we”) governing the ForLife Health ID app and app.forlifeglobal.health (the “Platform”). By creating an account you confirm you have read and agree to them. If you do not agree, do not use the Platform.
2. What ForLife is — and is not
ForLife is a digital health-data access facilitator: at your direction it retrieves results from authorised laboratories and health networks, renders them as signed Health ID cards, and lets you share a yes/no answer or card with people and partner apps you choose. ForLife is not a healthcare provider, medical facility, insurer, HIPAA covered entity or medical device. We do not generate, verify, edit or interpret medical records and do not provide diagnoses, treatment recommendations or medical advice. Records remain the responsibility of the originating provider; report errors to them. Always consult a qualified professional for medical decisions.
3. Eligibility and your account
- You must be 18 or older (or the age of majority in your jurisdiction) and provide accurate, current information, including the identity details needed for verification.
- You are responsible for keeping your credentials and device secure and for all activity under your account. Tell us immediately at security@forlifeglobal.health if you suspect unauthorised access.
- One account per person; you may not create an account for anyone else.
4. Your control over your data
Nothing is retrieved, shared or released to a partner without your explicit authorisation, which is time-limited, revocable and recorded in your audit trail. Partner apps receive only the signed aggregate answer you approved — never results, dates, identity or contact details. You may export your data (FHIR R4 / JSON) and delete your account at any time in the app or at /delete-account.
5. Acceptable use
You must not: access or attempt to access another person’s records; present a proof or card that is not yours or has been altered, revoked or expired; use the Platform to discriminate, harass, extort or coerce; probe, scrape, reverse-engineer or interfere with the service or its signing keys; or use it in breach of any law, including health-privacy and anti-discrimination law. We may suspend or terminate accounts that breach these Terms, with notice where lawful.
6. Proofs and verification
A ForLife proof is a signed statement that the holder’s records satisfied a stated question at a stated time and under a stated policy. It is not a diagnosis, a guarantee of current health status, or medical clearance; results have incubation windows and freshness limits. Verifiers must not store, redistribute or infer beyond the answer shown. Recipients in New York must comply with Article 27-F.
7. Partners
Organisations integrating the Partner Verification API do so under a separate partner agreement and the published API terms. Partner access is key-based, rate-limited, audited and revocable by both the user and ForLife.
8. Fees
The ForLife Health ID app is currently free for individuals. If paid plans are introduced, pricing, billing and cancellation terms will be shown before you are charged and you will be asked to accept them.
9. Availability and changes
We aim for high availability but the Platform may be unavailable for maintenance, upstream outages or events beyond our control. Do not rely on the Platform alone for urgent health needs. We may change or discontinue features; material changes to these Terms will be notified in the app and take effect no earlier than 14 days after notice unless required by law.
10. Intellectual property
The Platform, its software, designs, trade marks and documentation belong to ForLife Global Pty Ltd or its licensors. You receive a personal, non-transferable, revocable licence to use the app for its intended purpose. Your health data remains yours.
11. Liability
To the fullest extent permitted by law, ForLife is not liable for the accuracy, completeness or timeliness of third-party records, for decisions you or others make based on a proof or card, or for indirect, consequential or special loss. Our aggregate liability to you is limited to the greater of AUD 100 or the amount you paid us in the preceding 12 months. Nothing in these Terms excludes rights that cannot be excluded under the Australian Consumer Law or your local consumer law.
12. Governing law and disputes
These Terms are governed by the laws of New South Wales, Australia. We will first try to resolve any dispute informally within 30 days of written notice to legal@forlifeglobal.health. If unresolved, disputes are referred to binding arbitration administered by the Australian Centre for International Commercial Arbitration (ACICA), seated in Sydney, except that either party may seek urgent injunctive relief in court and consumers may bring claims in their local small-claims tribunal. Where permitted by law, claims are brought individually and not as a class.
13. General
If any provision is unenforceable the remainder stands. Failure to enforce a right is not a waiver. You may not assign these Terms; we may assign them to a successor of the business. These Terms and the Privacy Policy are the entire agreement. Sections 2, 6, 10, 11 and 12 survive termination.
